That’s why we’ve broken down the most common PennDOT and notary-related forms, explained when they are used, and outlined what you need to bring to complete the service at our office.
Reminder: State fees are set by PennDOT, but service fees apply when using a private messenger service like CNS Licensing.
Common PennDOT Forms
1. Affidavit of Gift (Form MV-13ST)
- When It’s Needed:
- Used when a vehicle is being transferred as a gift from one party to another.
- Helps PennDOT verify that the transfer is legitimate and whether sales tax applies.
- What You’ll Need to Do:
- Both giver and recipient must complete the form.
- Provide valid identification for both parties.
- Bring the original vehicle title.
- Proof of PA insurance in the new owner’s name.
- Step to Finish Service at CNS Licensing:
Visit our office with the required documentation and IDs. We’ll notarize the affidavit, complete the title transfer, and register the vehicle in the new owner’s name.
2. Affidavit of Fair Market Value (Form MV-3)
- When It’s Needed:
- Used when a vehicle is transferred without a traditional sale (such as between family members) to establish the vehicle’s fair market value.
- Helps determine applicable taxes and fees.
- What You’ll Need to Do:
- Fill in vehicle details and estimated fair market value.
- Provide identification and proof of ownership (title).
- Show proof of insurance for the new owner.
- Step to Finish Service:
We’ll notarize the affidavit, calculate taxes based on fair market value, and process the title transfer/registration.
3. Verification of VIN/Weights (Form MV-41)
- When It’s Needed:
- Required when transferring an out-of-state vehicle into PA.
- Used to verify the VIN number or correct discrepancies.
- Can also be required if weight class needs to be adjusted.
- What You’ll Need to Do:
- Download MV-41 and complete owner section.
- VIN verification must be done by:
- A licensed PA inspection mechanic, or
- A police officer from any jurisdiction, or
- In person at our office if you bring your vehicle with you.
- Step to Finish Service:
Bring the completed MV-41, original out-of-state title, PA license, and proof of insurance to CNS Licensing. We’ll process your title transfer and registration.
4. Assignment Upon Death of Owner (Form MV-39)
- When It’s Needed:
- Used to transfer ownership of a vehicle when the titled owner has passed away.
- Allows surviving spouse or heirs to claim ownership.
- What You’ll Need to Do:
- Provide a certified death certificate.
- Complete MV-39 form with estate information.
- Bring the original title (if available).
- Provide valid ID for heir or spouse.
- Step to Finish Service:
CNS Licensing will notarize MV-39 and handle the title transfer and registration into the new owner’s name.
5. Schedule of PennDOT Fees (Form MV-70S)
- When It’s Needed:
- Not a form you submit—this is a reference guide to PennDOT’s standard fees for titles, registration, plates, etc.
- How to Use It:
- Use this schedule to estimate PennDOT costs.
- Remember: CNS Licensing will also have a service fee for processing.
Miscellaneous Notary Forms
1. Letter of Authorization – Corporation
- When It’s Needed:
- Used when a company authorizes someone else to sign PennDOT paperwork on its behalf.
- What You’ll Need to Do:
- Business letter on official letterhead.
- Signed by an officer of the company or owner.
- Include name and ID of the person authorized to act.
- Step to Finish Service:
Bring the signed authorization letter, IDs, and any necessary PennDOT forms. We’ll notarize and process the transaction.
2. Motor Vehicle Bill of Sale
- When It’s Needed:
- Documents a private sale of a vehicle.
- Used when a title transfer requires proof of purchase price.
- What You’ll Need to Do:
- Complete a bill of sale with buyer and seller names, vehicle details, sale price, and signatures.
- Provide the original title, IDs for both parties, and proof of insurance.
- Step to Finish Service:
We’ll notarize the bill of sale and complete the transfer of title and registration.
